• Offering support coordination within a robust specialist framework
• Collaborating with participants, family members, and key stakeholders
• Identifying and addressing both immediate and ongoing risk issues
• Conducting a thorough needs assessment and creating an implementation plan
• Delivering intensive case management support
• Performing initial crisis assessments
• Coordinating responses from emergency services, mainstream services, and NDIS supports that enhance capacity and resilience, reducing reliance on intensive supports
• Facilitating access to therapeutic supports
• Overseeing the implementation of an individual’s NDIA plan, including monitoring spending and service quality
• Addressing complex disability-related needs that necessitate comprehensive liaison with various services
• Exploring and sourcing appropriate services to meet individual support requirements
• Providing comprehensive written reports, including referrals and progress updates to NDIS
• Building and nurturing strong connections with NDIA and relevant agencies to ensure participant needs are effectively addressed
• Assisting in exploring housing options, gathering relevant assessment reports, and liaising with NDIA and service providers during the Supported Independent Living (SIL) application process.

Participants can utilize the support coordination and/or specialist support coordination funding outlined in their plan to access these services.
